Providing patient advocate and case management services in the nature of healthcare navigation for patients, namely, coordinating patient access to healthcare and medical treatment information, coordinating the procurement and administration of key healthcare services and related healthcare management tools to further patient treatment, coordinating the sharing and transmission of patient case information between stakeholders, and coordinating communications and collaborations with patient stakeholders in the delivery of personal care for the patient; Providing patient advocate and case management services, namely, coordinating the procurement and administration of healthcare treatments by assisting patients and their representatives in accessing safe, compassionate, competent, and ethical healthcare; Providing personal support services in the nature of caregiver relief and caregiver backup services by accompanying patients to medical and dental appointments, health treatments and procedures, and emergency room visits and discharge; Providing patient advocacy services in the nature of patient shadowing services, namely, following patients and their representatives throughout selected healthcare experiences to view, observe, record, capture, and evaluate the entire healthcare experience; Providing patient advocacy services, namely, providing support to patients by monitoring their experiences via telephone an drop-in visits, and coordinating patient navigation to receive health treatment and medications
